Level 1 No change in patient outcome
Level 2 Increased monitoring required; therapy changed/held/discontinued
Level 3 Additional labs or diagnostic tests were ordered; vital signs changed
Level 4 Reason for admission; prolonged length of stay
Level 5 Transfer to ICU; monitored bed required; invasive procedure required
Level 6 Patient death
Recoding levels 0–1 = mild severity
  levels 2–3 = moderate severity
  levels 4–6 = substantial severity
